By signing this application, you are entering into an agreement that LMUD provide
your electric service and accept our rules and regulations.
LMUD can generally provide next day service if arrangements are made before
4 p.m. However, to assure that your service is connected when you need it,
please make arrangements several days ahead.
Please take the time
to provide all requested information to insure that your request
can be expedited quickly.
Prior
to receiving electric service the customer shall provide the
LMUD with one of the following means of insuring payment:
Confirmation and acceptance
of credit history through verification with one or more of
the major credit reporting agencies. If credit history is not
acceptable, or cannot be verified, customer will be required
make a deposit as set forth herein. Approval of waiver
of cash deposit or equivalent is exclusively within the discretion
of LMUD.
Deposit equal to two-months
average consumption (or estimated consumption based on design
of service) with a minimum of $100.00 for residential accounts;
Deposit equal to
two months average consumption (or estimated consumption based
on design of service) for a large commercial and agriculture
pump accounts;
Two months average
consumption (or estimated consumption based on design of service)
for small commercial accounts;
Business, agricultural
pumping, and commercial accounts may provide a surety bond or
other acceptable form of non-cash deposit in lieu of a cash deposit
upon District approval if the following criteria are met:
1. Surety bond is written
by a reputable bond company licensed in California and countersigned by a registered
agent of the bonding company. Such bond to be reviewed
annually and must contain a minimum 60 day cancellation clause. Written
notice of renewal must be provided to LMUD at least sixty (60)
days prior to the expiration date of the bond. In the
event of cancellation of the surety bond, Customer will provide
District
another form of deposit in accordance with the terms herein
or may be subject to disconnection of service upon 48 hour
notice.
2. Accounts
with an average monthly consumption exceeding $10,000 may be
required to post a bond in accordance with the provisions set
forth above.
This
deposit is held for one calendar year.
At the end of the year, the deposit will be applied if the account has remained
current. If the account is closed prior to one year, the deposit will be applied
to the closing statement and any credit balance will be refunded by check.
Deposits may be made using VISA or Mastercard. Please note there is a
small convenience fee for all credit card transactions.
LMUD
may disconnect or refuse to provide service to the applicant
if the acts of the applicant or the conditions upon the customer's
premises indicate that false, incomplete, or inaccurate information
was provided to LMUD. LMUD shall provide the applicant the
reason for such refusal.
Individual
Liability for Joint Service: Two or more parties who join
in one application for service shall be jointly and severally
liable thereunder and shall be billed by means of single monthly
bills. Whether or not the utility obtained a joint application
for service, where 2 or more adults are living in the same residence,
they shall be jointly and severally liable for bills for energy
supplied.

To begin receiving electric service from LMUD, please visit our office, located at 65 S. Roop Street, Susanville, as soon as possible in order to sign an Application for Service.
