A cross-section of LMUD customers will be contacted by email and given a secure link to the survey, where they will answer various questions regarding LMUD. You can scan this QR code if you did not receive an email invitation but would like to participate. The code will take you to the survey, where you willneed your LMUD account number to complete the questionnaire.
